Prescription Medications
If your student will need to take any prescription and only prescriptions during the overnight trip. Please fill out the MAR which is attached, have it signed by the parent, bring the filled out and signed form and the EXACT number of pills, in the original prescription bottle to the nurse on your student’s campus. If your student is in 9th or 10th grade that is Nurse Lambert, if your student is 11th and 12th grade that is Nurse Greenberg. Again the pills have to be in the original bottle, the nurses cannot accept anything that is not in the original prescription bottle. If your student self carries medication due to severe allergies/potential anaphylaxis, diabetes etc. That information should’ve been included already in your fieldtrip parent release form.
